On testing day, ProAct’s Concussion Team will collect the following forms prior to sitting for the test. Please fill out the forms below or bring the form that you may download below with you on the day of the event.  Every test administrator is a HIPAA compliant healthcare provider that has been trained by ProAct in concussion test administration.
